Application for a Reiki Treatment/Consultation |

| Before you receive your first Reiki treatment with Matthew, you will need to complete an application form. Right-click on this link: FORM.RTF and choose 'Save Target As' to store the file in an appropriate folder on your PC. The file may then be loaded into your local word processor. Please print the file and bring it along on your first day. Alternatively, the completed file may be emailed to and you will be asked to sign it when you arrive. |

| Matthew wants all his clients to be delighted with their treatments and courses. Personal recommendation is far more important than advertising. In light of this Matthew rewards clients who recommend his services to their friends, family and colleagues. If your name is mentioned when a booking is made then you will be rewarded with a voucher - £5 if you’ve recommended someone for a treatment and £10 if it’s a course. All vouchers last for six months and can be used against treatments or courses. Recommend just a few friends and you’ll get a free treatment or be well on the way to funding a course. |
